1st Pay the CAA application fee ($121).
CAA require that the CAA fee is paid prior to your appointment. This can be done online in Sky Cert via the CAA website.

Part 67 - Medical Certificate Application Fee
The medical certificate application fee must be paid before your medical examination.
CAA does not require a payment for NZTA DL9 Medical Certificates
The medical certificate application fee is not refundable.
Your Medical Examiner will require the receipt number at the time of booking your medical examination appointment. NB: Your receipt will be emailed to you following payment.
If you are living overseas, GST may be excluded from the transaction (see link on international tax residency).
Nominate a CAA medical examiner (see link for list of CAA medical examiners).

Plus if necessary…
Spirometry $95 ( Lung Function Test)
ECG $80 ( Heart activity test)
Extension certificate (60 days) $ 132 (incl GST)
A urine test is required for testing at appointment time.
Depending on what else you need, and where you go, other costs are likely to be as below:
Lab tests approx. $70-$80 (if you have had fasting lipids and glucose/HbA1c by your own GP in the last 11 months you can bring a printout of these results instead).
Optometrist report (class 1 and 3 only): $100–$200
Audiologist report (not required for Class 2 if not IFR): $80-$150
Chest X-ray if any lung problems: approx. $100–150
Cardiologist report on ECG if there are abnormalities: ?
Blood tests may be charged at the lab…
Other tests or consultants fees, if needed…

Complete your online SkyCert application
Please supply your CAA participant number. If you do not have a CAA participant number, your medical examiner will apply for one.
Bring – glasses, spare glasses, contact lenses (but come with contact lenses out).
Bring photo ID (e.g. drivers licence, passport)
Bring a copy of your receipt of payment from the CAA or have it forwarded to me by email
